Request personal records

We make it easy to request medical records, whether you are a patient, a healthcare provider, or a third party acting on a patient’s behalf.

Who can request records

  • Patients requesting personal records
  • Healthcare providers
  • Third parties, such as insurance companies, workers’ compensation carriers,
    disability agencies, or attorneys

How to request records

All medical record requests use the same form.

  1. Download and complete the SMS Authorization for Release of Information form listed below.
  2. Return the completed form to Health Information Management via the following methods:

Please note:

  • Requests must be fully completed to avoid delays.
  • Fees may apply for copies of medical records, as permitted by law.

Request medical records for deceased patients

To obtain a copy of a deceased patient's medical record:

  • The executor or administrator of the estate must complete, date, sign and return the SMS Authorization for Release of Information form linked above. A copy of the will or court order must accompany the completed form.
  • If you are requesting a deceased patient's records for purposes other than settling personal or financial affairs of the descendant, contact Health Information Management. Please see the Contact Us information below.
